This form allows you to submit concerns regarding the welfare of animals used for teaching or research at RRC Polytech. All submissions are reviewed by the Veterinary Technology Program Chair and the Associate Dean, School of Health Sciences and Community Services, who will inform the Animal Care Committee (ACC) in accordance with policy R3 - Animal Care and Research.

Your identity and any supporting information will be treated as confidential, and access to your submission will be restricted to the authorized Animal Care Committee (ACC) and veterinary personnel.
